Headaches and migraines are among the most common medical complaints, yet they are also one of the most challenging conditions to diagnose and treat appropriately. There are a variety of reasons why doctors may struggle to identify the cause of headaches and migraines, but four common reasons include:
Lack of specific training in assessing the upper cervical spine: Most doctors are not trained to evaluate the upper cervical spine (C0-3) when investigating headaches and migraines. This can lead to a failure to correctly identify cervicogenic headaches, which are caused by problems in the neck. This is a common but often overlooked cause of headaches and migraines. In twenty-plus years of practice, I've never had a patient who suffers from headaches tell me their doctor laid them face down and specifically examined their upper three cervical vertebrae and that the doctor could determine if their headaches were influenced by their cervical spine. If you've never had your upper cervical spine examined and suffer from headaches, you owe it to yourself to seek specialized care.
Overlap of symptoms between different types of headaches and migraines: There are many kinds of headaches and migraines, each with distinct symptoms. However, there is often an overlap between the symptoms of different types of headaches and migraines, making it difficult for doctors to diagnose the condition accurately. This is particularly true for tension-type headaches, which can often be mistaken for migraines.
Lack of reliable diagnostic tests: Unfortunately, there are currently no medical tests or investigations that can be used to confirm a diagnosis of headaches or migraines. Instead, diagnosis is based on the patient's symptoms and a classification system established by the International Headache Society. This system has been criticized for being too large, unwieldy, and complicated for clinical use.
The possibility of multiple underlying causes: Headaches and migraines can be caused by various underlying conditions, including neck disorders, stress, depression, and hormonal imbalances. This makes it difficult for doctors to pinpoint the exact cause of a patient's headaches and migraines. Additionally, many patients experience multiple underlying causes, further complicating the diagnosis and treatment process.
